In-Network With Select Plans
We’re primarily cash-based and now in-network with a select few plans (currently Cigna, Aetna, and Curative), which we bill directly. For other plans we’re out-of-network — appointments built around your care, not insurance quotas.
Superbills for Reimbursement
After your visit, you receive a comprehensive superbill to submit to your health plan for reimbursement — we make the paperwork easy.

More plans may be added over time. Don’t see yours? You can still be seen as a self-pay patient — we’ll provide a superbill to submit for possible out-of-network reimbursement.

Ellie Frazey, M.D.
Board-certified neurologist with nearly a decade of clinical training and experience in neurology. A dedicated Tampa Bay local, Dr. Frazey completed her undergraduate studies, medical degree, and neurology residency at the University of South Florida — earning her M.D. from USF’s Morsani College of Medicine, a Tier 1 research medical school (U.S. News & World Report). She brings a unique perspective to brain health.

Do you offer telemedicine?
Yes. We offer secure virtual visits for new and returning patients. Simply choose an in-person or virtual appointment when you book online.
Do you take insurance?
We're in-network with a select few plans — currently Cigna, Aetna, and Curative — which we bill directly, and we expect to add more over time. For all other plans we remain out-of-network: after your visit you'll receive a comprehensive superbill to submit for reimbursement.
What conditions do you treat?

How do I become a patient?
Book directly online through our scheduling page, or have your physician fax a referral to our secure eFax at (949) 864-3647. Urgent consultations are typically accommodated within 48 to 72 hours.
What should I bring to my first visit?
Please bring a photo ID and your insurance card (for your superbill), a list of your current medications and dosages, and any relevant medical records — including prior imaging (MRI/CT), lab results, and notes from your referring or primary physician. It also helps to jot down your main symptoms, questions, and concerns beforehand.
